Application Form - This form must be complete to be eligible for Creative Kids funding.
Service Provider Form - All individuals, businesses, and organizations providing a service to a Creative Kids applicant must complete and sign this form before funds will be released
Donation Form - We are able to accept donations via cash, cheque and credit card. Any donation $20.00 and over is eligible to receive an official tax receipt.
